Please note: Birthday boxes are designed only for inpatient facilities or pediatric oncology clinics on the child’s day of treatment. Unfortunately, we cannot provide boxes for in-home care.
Who Can Submit This Form
This form is for Child Life Specialists only.
Each hospital may only have one Child Life Services department participate (exception: if the hospital also has an oncology clinic).
Important Timeline
We currently have a 9+ month waiting list to add new or returning hospitals.
Submitting this form does not guarantee immediate participation, but it places your facility on our waitlist.
Once accepted, your facility needs to order a minimum of 2 times per year to stay active (Jan-Jun and July-Dec)

Each birthday box is designed to be delivered on the child’s actual birthday, ensuring a magical celebration.
